Construction of the extension building for our faculty

The construction of theextension building for PAPHRSB Institute of Health Science hascommenced on Thursday, 6^th October 2011. Theexpected date of completion is 5^th June 2013 witha construction period of 20 months. The project cost approximatelyBND23 million and will be constructed by Tobishima Sdn Bhd lead by a team ofconsultants from Architect Idris (Lead), Othman and Associates (Mechanical& Electrical and Civil &Structural) and Hanafi Consultant(Quantity Surveyor). The new extension building is located within the Scienceand Technology Triangle, next to the University Technology Hub and the existingPAPHRSB IHS existing Building.

This isthe fourth of six projects to be rolled out under the currentNational Development Plan. The extension building will house more officesfor researchers and staff, a research lab, several teaching labs, lecture hallsand a Health Centre [further details on its facilities can be obtainedfrom Estate Office].

Synopsis

PowerPoint presentations are very popular in delivering lectures and other materials. Millions of copies of this program have been sold all around the world. There is no doubt that it helps the speaker or lecturer to assemble professional looking slides and to deliver the material to audience without fear of missing something important. In short, PowerPoint makes life easy for the presenter. With enormous scientific development and increasing complexity of subjects, the amount of information needed to be delivered has also increased and the result is often an unending stream of slides with loads of information in the form of bullet lists. Animations, designs, colours, font sizes, styles and cartoons are used in order to make presentation more interesting and appealing for the audience.

In recent years, the phrase `Death by PowerPoint` has become popular in describing the adverse effects of badly prepared PowerPoint presentations. Studies have shown that animations obscure rather than clarify the point. Similarly, the cartoons used in slides usually distract rather than convey the message. Overall the format of presentation becomes more important than the content.

The purpose of this lecture is to discuss the advantages and disadvantages of using PowerPoint based on research and evidence; and to identify the ways to enhance understanding and deep learning by presenting material in a way that helps in information processing rather than mere information presentation.

Open Day At PAPRSB Institute Of Health Sciences, UBD

Bandar Seri Begawan - The Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah Institute of Health Sciences (PAPRSB IHS), Universiti Brunei Darussalam held its Open Day yesterday.

The PAPRSB IHS open day was held in conjunction with the celebration of Universiti Brunei Darussalam's (UBD) upcoming convocation for the year 2011.

Holding the Open Day for the first time, PAPRSB IHS chose 'Bring Your Heart to IHS' to commemorate the World Health Day and held for one day only.

In commemoration with the upcoming World Heart Day on September 26, the institute will be showcasing exhibitions, promotion of healthy diet as well as holding quizzes and talks.

The Open Day also offered prospective students the opportunity to gain an insight into life as a student at the institute.

Moreover, the IHS also displayed their teaching and research facilities, while medical, nursing and midwifery students showcased their healthcare skills.

During the Open Day, the students came from secondary schools and colleges, had the opportunity to tour the institute and the IHS promotion booth to learn more in depth on the health science programmes that are being offered. The students also attended a talk presented by the IHS students.

A free medical check-up and health advice booth also set up to offer medical consultancy throughout the day. Health talks by invited medical professional from Health Promotion Centre and light physical activities were also part of the Open Day activities.

Convo Run & Fun Cycling

Universiti Brunei Darussalam will be organizing a Convo Run and Fun Cycling event on Sunday, 2 October 2011 as part of its Pesta Konvo 2011.

Participations are open to members of the public with a registration fee of $15 per person. Registered participants will be given a Convo Run and Fun Cycling T-Shirt, and a certificate of participation.

Those interested can only register one event either the Convo Run or the Fun Cycling.

The Convo Run will cover a distance of approximately 6km which begins and ends at the Core, UBD New Residential College. Whereas, the Fun Cycling route which covers an approximate distance of 35km will begin from the UNISSA Football Field car park at Jalan Pasar Baharu Gadong and cycle all the way to the Core UBD New Residential College.

The Convo Run will be divided into two categories – Male and Female. 1st place will receive $200, 1st runner up will get $150, and 2nd runner up will receive $100. However all top ten finishers will be given a ‘Top 10 finishers medal’. The Fun Cycling is a non-competitive event hence no prizes will be given.

Registration can be made at the 1st Floor Resource Room, Student Affairs Building UBD during office hours beginning from Monday, 19 September 2011. Registration form can also be obtained at the following outlets; PMUBD Shop, Gill Sports in Kiulap and Batu Bersurat, Mufakat Trading Co. in Gadong, Imperial Trade and Co. in Jalan Batu 1, and Triple Star Enterprise in Kiulap. Registration booth will also be opened during race day at both venues and will close at 6:45am.

How old are you? - I’m 28 yrs old

Which part of Ghana are you from? - I am from northern Ghana but have lived exclusively in the South

What do your friends usually call you? - Here in Brunei, I think it is Abdul or Abang courtesy my adik-adik

Describe yourself in 3 words. - Kind, Considerate and Fun

What is your favourite quote? - “If you find or see something you don’t like in a brother, try to find 1-70 excuses for him. And if you can’t find an excuse, say ‘there might be an excuse, but I don’t know it’”. Imam Jafar as-Saadiq

What is your hobby? - Well I got a few; Reading, learning new languages (I can speak 5 fluently as well as write and make random sentences in 2 others), Martial Arts and cooking.

Do you have anyone that inspires you? - The Beloved Prophet Mohammed (May peace and blessings of Allah be upon him and his household)

Abdulai’s opinions

What is the most important thing in life? - Love because I believe it is what makes the ride of our worldly life worthwhile.

If you have 1 million, what would you do with it? - I would invest in establishing a “Mobile Clinic” comprised of personnel from the various medical departments and basic equipments. Please give me a call when you write that cheque!

If you have one way to help the others, what would that be? - I have seen plenty people, mainly kids living in rural areas and when sick have to wait through their sicknesses due to limited access to healthcare or its lack thereof for various reasons; some recover but many others succumb to their ailments. This is devastating to their parents and it breaks my heart. I would reach out to these people and bring to them at least basic healthcare through a mobile clinic.

If you were given one chance to reverse time, when would it be and why? - I think it would be sometime in 2004. This is because the iron was red hot but I failed to strike.

If you could build a house anywhere, where would that be? - In my home country Ghana!

Abdulai on personal matter

Where do you want to go on vacation? Why? - Cook Islands in South Pacific, because it is exotic with beautiful beaches, quiet and peaceful atmosphere. Besides, I find the names of the islands quite interesting and I think it would be nice to hear the local people speak and to learn a few phrases too! And hey they got lots of bananas there!

What is your favourite TV series? - F.R.I.E.N.D.S

What is the best and the worst movie(s) to you? - You know what, I suck at judging other people’s efforts and I believe every movie has its own best and worst part.

Do you have any favourite restaurants here in Brunei? - Hmmmm, I can’t think of any at the moment, because I hardly eat from outside.

Do you have any favourite meals? - Yeah! Fufu and light soup, Cucur pisang, Jollof Rice and Nasi Ayam penyet, yeay.

Abdulai as a biomedical student at IHS

How is university life so far? - It’s been quite challenging but exciting.

Describe your life as a biomedical student. - Going for lectures, reading, doing LOBS (well not anymore) writing essays, projects and laboratory placements at the hospital and lab sessions in IHS.

What is your favourite biomedical subject? - That would be Immunology; I find the intricacies of the immune system simply amazing!

Describe your best moments so far in IHS? - My first Hari Raya Eid-ul-Fitr in IHS 2009, I got to wear the Cara Melayu a friend lent me and also stepped to the Hari Raya song, everybody was so happy and friendly, it was fun.

Abdulai as a future biomedical scientist

Why do you want to become a scientist? Do you have any inspirations? - I want to become scientist to join in the search for cures and vaccines to infectious diseases. My inspiration comes from the realization that a lot of effort and money is being pumped into discovery of drugs and vaccines to diseases and to make these affordable or even free to people. I come from an area where some of these diseases are a burden so I am inspired to join in this quest and hopefully in the near future there will be the discovery of cures to some of these devastating infectious diseases which shall be affordable to all who would need them.

Have you thought about any specialty of interest at the moment? - Yes I have and it will possibly be in Infectious diseases, Vaccinology and Drug discovery.

Any study tips for us? - For whatever subject you are studying, just find one recommended textbook, then shut up and read! Don’t just study, but read, memorize stuff you need to, soak it up and digest the information to build a good knowledge foundation from that one book. Then look up extra information from other sources to strengthen that foundation and build on. Discuss with colleagues and explain to them if necessary. Relax, review and integrate the information and then find your own voice by writing down in your own words and most importantly, learn to remember. You will be able to argue and answer your questions better. Just leave the awarding of marks to the lecturer people.

Any advice/word of success to the juniors? - Just know yourself, know your capabilities and your limitations and don’t fool yourself. Find something you are genuinely interested in and pursue it with all your might and move as if you will never fall but when you do fall, which of course you will occasionally, get up brush yourself down, look where you tripped, remove the stumbling block and caution those behind you and continue moving as if nothing happened having faith solely in Allah and trusting Him completely. You will reach there and not only you, you would have helped others too!

Adhwa as a biomedical student at IHS

Describe your life as a biomedical student. I studied health sciences together with Medicine Cohort 4 throughout my first year, and then studied modules specialised in biomedical sciences such Clinical Microbiology (Bacteriology, Virology, Mycology and Parasitology), Histopathology, Medical Genetics, Clinical Immunology and more for the next two years. Also, as partial GenNext undergaduates, I also took external modules (which are usually our time to clear our minds from health sciences) such as Children’s Literature and Maths & Technology. Then being in a special first cohort, we did our academic final year in the third year, allowing us to do our Research Projects (my most heavy-reading and time-spent module!) throughout the year. Then in the final year, we will be heading for our Discovery Year. Not forgetting, my cohort only has 11 people, and being a small group makes us close to each other as family.

What do you think about IHS? IHS has taught me so many things, academically and personally. It is a place where I really learnt independence. The admin and teaching staffs, also the technicians are very friendly and approachable, making my university life worth it.

What is your favourite subject(s)? Firstly, I like Anatomy in my first year because it is my opportunity to draw stuffs on my notes and even on the whiteboards (and trust me, I even drew on my exam paper too!) Apart from that, I like it for the sessions that we have practically, which involved organ dissections. And then also, recently I am in love with Research Project as we were given the chance to perform our experiments by ourselves. It is the hardest module I ever had so far as a biomedical scientist, but we survived impressively!

Describe your happiest moments IHS. My cohort has this tradition that we started in the first semester – we went out for movies, ambuyat, or beach on the exact day of our final exam paper. It’s like our run of freedom. It’s funny that during the exam week, we seemed to be more excited about the tradition outing than to worry about our final paper! In addition, there is another tradition involving the whole faculty in which the juniors must perform during the seniors’ graduation night – I performed for two senior cohorts! The dance rehearsals and performances were memorable and full of laughter. I never qualified myself as a dance expert, but I had fun anyway!

Where do you plan to go to in the final year? Krankenhaus Nordwest, Frankfurt, and Otto-Meyerhof-Zentrum, University of Heidelberg, in Germany

What branch of biomedical science you want to purse? To be honest, I am not yet sure. I might go for Public Health or even, Experimental Research. But you may never know, since I will be going to a Neuroscience based institute, I might eventually choosing that path. My wildest dream would be to be a designer for the Anatomy models and research experiments – if it ever exists!

Any advice/words of success to the juniors? Just express yourself. Be heard, and love your coursemates!

UBD participated in the 7th Malaysia Indonesia Brunei Medical Sciences Conference 2011, with the theme 'Towards a holistic and integrative approach in healthcare'. This year, the conference was held in Equatorial Hotel, Bangi, Selangor, Malaysia, from the 22nd July 2011 until 24th July 2011. It was officiated by Yang Berhormat Datuk Rosnah Haji Abdul Rashid Shirlin.

Those who participated includes our current IHS Dean Dr Maslina, our Dean of Students Affairs Dk Dr Nurolaini Pg Hj Muhd Kifli, our IHS Deputy Dean Dr Mohd Ayub Sadiq @ Lin Naing, Dr Tayyab Hassan, Ampuan Dr Hj Asrin Ampuan Hj Tengah, Dr Fazean Irdayati Idris and Azim Matnoor. Also participated are the students of IHS: a medical student named Hj Muhammad Ainuddin Hj Mahli and four Biomedical Sciences students namely Fakhriedzwan Fitri Hj Idris, Zaharuddin Haji Abdul Rahman, Siti Nur Bazilah Ghazali and Nurul Adhwa Hj Abd Rahman. SIX medical students from Japan's Kagawa University yesterday began a five-week course at the Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah (PAPRSB) Institute of Health Science, Universiti Brunei Darussalam (UBD).

The students are participating in the Fifth International Summer Medical School Programme 2011, an annual exchange programme between PAPRSB Institute of Health Science and Kagawa University's Faculty of Medicine.

Dr Suwarni Hj Mohd Diah, the programme chairperson, said the participants will attend lectures, problem-based learning sessions, clinical and communication skills, laboratory sessions and tutorials during the course.

"Participants will also have a chance to gain knowledge and hands-on experiences from clinical placements in health centres and hospitals throughout Brunei," she said.

She went on to say that all sessions will be conducted in English as this will provide the opportunity for Japanese students to enhance their English language proficiency as well as their communications skills.

In addition, the participants may also have the opportunity to learn about Brunei's way of life and culture through staying with foster family and visits to various cultural and heritage places such as Kampong Ayer and the Royal Regalia building.

They will also visit the Belait district and experience Brunei's unspoiled natural heritage by going to the Kuala Belalong Field Study Centre in the remote area of Temburong.

"This year, participants will have the unique opportunity to experience the special, spiritual and social atmosphere of the holy month of Ramadhan," said Dr Suwarni.

She hoped that all Japanese students in the programme would enjoy their stay and bring a lot of good memories from their experiences in Brunei.

At the end of the course, participants will be assessed by a written examination and Objective Structured Clinical Examination (OSCE).

The students are also assessed on their presentations and report-writing skills.

Yesterday on the 9th July 2011, an event was held at the institute, welcoming the first cohort after successfully graduated from their partner medical schools. Impressively they are doctors now. The fresh graduates went for a three-year programme at the institute to obtain an ordinary degree and another three years abroad. The event to celebrate the first UBD produced doctors was attended by the Vice Chancellor, Dr Hj Zulkarnain Hj Hanafi, who was also a former Dean of the institute.

Dr Hjh Maslina Hj Mohsin, the current Dean, in her welcoming address said the team is very proud of this achievement and also mentioned few other great achievements of the cohort, including the international inter-medical school physiology quiz, Institute of Medicine Charity Evening for Smarter as well as several medical school exchange programmes.

The new graduates mentioned they were glad that they joined the institute (formerly known as Institute of Medicine) before they went abroad and were very grateful that the team has prepared them very well. The doctors, in their presentation, shared their experiences at their partner medical schools and gave useful advices for the juniors who will be leaving for their respective partner medical schools.

Dr Hj Zulkarnain, in his speech, mentioned that the journey matters more than the destination. He also added that perseverance with a very clear directional vision and effort helps too. He reminded the new graduates that first and foremost you are human, secondly you are a doctor and only thirdly you are a surgeon.

Describe your life as a medical student. – difficult at times, but overall fun, and a great learning experience What is your favourite medical subject? – so far the neurology module was cool

What is your favourite PBL case so far? – I don’t even remember cases that far back, I’ll go with multiple sclerosis, that PBL session went better than most

Among all of your cohort 5 colleagues, who is your best buddy? – hard to name 1 :P (sorry Mizah, the rest will kill me)

Describe your best moments so far in IHS? – Wasn't in IHS but the KK trip last year was pretty sweet

Where are you planning to go after this? – I haven’t decided which partner medical school yet, but eventually I would like to end up back in Canada

Any study tips for us? – Make your own very compressed notes so you don’t have to go back to the book, makes studying more efficient. And write things down over and over and over again without looking to help in memorization.

Any advice/word of success to the juniors? – it might be hard right now but its only gonna get harder. Enjoy it.

Haha… I refer this group of people as ‘great people’ in my life. They come and go in my life. For now, I have two. One was from IHS. One is from IHS. They inspired me to be a better person. I am working to be similar to them. Different person inspired me in different area. I guess, people who know me well will know who my great people are. Go figure!

Describe your life as a medical student.

I have to say it’s nothing of interest until I became active. In second year, I was a hardworking student. I studied everyday, though minimal but it was the reason why I managed to maintain a grade B. In second half of second year, I started to become active. This was the time all the fun came in. I have to balance between my studies and my activities.

I have learned a lot as a medical student. I have presented a lot of time. Gained confidence. Went to Japan. Medicine course is building my character in a good way.

What do you think about IHS?

Personally, IHS is a place where I was being nurtured into a better person. Though I have to admit that the process will continue.

If I am given a chance to re-choose my choice to enter UBD, I will choose UBD still. I never regret the decision of entering Medical Course in UBD, though I was peer pressured into believing that ‘I must go abroad after A’Level’. I believed most would have this attitude.

Describe your happiest moments IHS.

The scene that I remembered the most was when my batch was in first year. Then, we had to move from place to place. For instance, we would be having Expert Forum in Chancellor Hall (CH) basement room. Right after that, we had to go to the old IM’s building. Due to limited parking spaces (students usually park on the bus stand!), we would fight for parking by racing each other out of CH. I would laugh at the scene where we had to run out of CH towards the car, though I was not driving then. I enjoyed that moment. How I wish I could still relive that moment.

What is the most important thing in life?

Life itself is an amazing gift. That is why I am in the field of dealing with life biologically.

I think mentality is very very important for me. It defines one’s outlook of life which eventually lead to how a person would handle things. I think I am fascinated with this.

If you have one thing to change in this world, what would it be and why?

If one is in a beauty pageant, one would answer, world peace *wave the hand with pride*. Personally, there is nothing in my mind.

If you have one million dollar, what would you do?

Ha! I will keep it in the bank and use it in time of needs. Anyhow, when this happens, I am sure there will be people advising me on how to use the money. Actually, I don’t really think about this.

If you were given one chance to turn back time, when would it be and why?

In life, time waits for no man. Once the time has passed, it’s a past. Live life to the fullest, so that there will be no regrets. The reason I answered this way is because I don’t know when and why I would turn back in time.

Why do you want to be a doctor?

This is a million dollar question. Always being asked. My reply has matured as I answered the question repetitively.

As a young child, I love to watch tv. I am always amazed by how a doctor could handle stress. In a stressful situation, he or she would be able to make decision in the nick of time. A doctor deals with life that I think it’s more valuable than any other thing the world has to offer (especially referring to money).

From young, I always have this feeling of excitement and heart pounding whenever I watch or being exposed to the health field. The feeling is just like when one meets the love of his/her life. You just know at that very instance that it will be your life and there is no such need for a reasoning anymore.

Which area do you want to specialize in?

I have a few interests currently that include Accident and Emergency Department, and Child Psychiatry (more like Psychology rather than Psychiatry). However, anything could change after this.

What will you miss most about IHS?

I will miss everything, especially my friends and lecturers.

Any advice/words of success to the juniors?

First, cope with the studies. Once that is done, then explore your environment! UBD offers a lot of opportunity to go abroad! Grab the chance while you can! Just be yourself!

Study smart, play hard.

Time management and motivation will be the answer.

Few weeks ago on the 23rd April, the Earth Day Green Race was organised by students taking the GenNext breadth module - Environment, Society and Globalisation (ESG). The purpose of the event was to increase awareness among students and public members alike of the existence of Earth Day.

More than 50 undergraduates participated in the race.The participants were to move/run around the campus in order to complete different challenging tasks for example cycling, making paper butterflies, counting car parks at IHS, looking for clues in bottles, etc. There were 12 groups altogether, each having 4 members. Two of the groups were joined by IHS students.

13th April 2011- 4th Annual General Meeting has been held today at Chancellor Hall. It discussed about the problems faced by UBD students and to show proposals made by PMUBD.

The event started with the arrival of guest of honour, Yang Mulia Prof Madya Dr. Azman bin Ahmad, assistant of deputy chancellor UBD.

LOCAL NEWS: PAPRSB IHS To Have New Buildings

The Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah Institute of Health Sciences (PAPRSB IHS) is building new extension buildings that will triple its current size. Dr Hajah Maslina binti Hj Mohsin, Dean of PAPRSB IHS, said the new building would include new lecture rooms, theatres, clinic, and health centre and incorporate a simulation area for health students at the institute.

In relation to the clinic, she said the patients will be the students in the university, including those in other faculties. However, if there is a need to extend the services to the community in the area, it will be possible, but naturally it would not be logical to overlap services from the Ministry of Health in that area.

Asked about the possibility in the future to eventually set up a university hospital. Dr Hjh Maslina, who is also the Director of Health Services in the country, said setting up a university hospital is a totally new ball game. But Brunei is hoping to build a second hospital in Brunei-Muara District, and the proposed area might not be far from the university campus.

Dr Mohd Ayub Sadiq @ Lin Naing, the Deputy Dean of PAPRSB IHS said, with the new extension, it is now possible for the institute to provide the students with simulation wards and incorporate real patient clinic. The institute is attracting students locally as well as from abroad. He said there is still room for more students to enrol in the institute. It is important to educate students that health services in a noble profession. The emergence of "centres of excellence" like the Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah Institute of Health Sciences will not be "at the expense of" the institutions in the UK, Mr Rob Fenn, the British High Commissioner said yesterday during his visit.

These institutions in the UK have for so long attracted Bruneian students, he said. "Public health and education occupy strategic positions in Brunei's national life, and are areas where the UK and Brunei can take their longstanding partnership to new levels. "I am a strong supporter of human capital development here in Brunei, as well as in the UK. This is not a zero sum game … quite the reverse, especially in highly technical areas such as medicine."

"The fact that Brunei's ministers of health and education have both visited the UK this year to explore partnerships with British universities and teaching hospitals underscores the fact that we are together looking for new ways to strengthen Brunei's public health infrastructure. I hope this can extend to areas such as improved IT and management information systems, drawing on British expertise in that key area," he said.

During the visit, the British official was briefed by the Dean of PAPRSB IHS, Dr Hajah Maslina, who is also the Director of Health Services, Ministry of Health. The British high commissioner also heard from Dr Mohd Ayub Sadiq @ Lin Naing, the Deputy Dean of PAPRSB and Hjh Jainah binti Hj Musa, progamme Leader of Nursing and Midwifery.

AWARENESS on the importance of pap smear screening in Brunei is low amid the world's concern on the high number of cervical cancer which is the second most common malignancy in women.

This was part of the outcome of a research prepared by two graduate students of the Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah Institute of Health Sciences (PAPRSB IHS), Mohammad Noh Hj Latip and Muhamad Ibnu Walid Hj Adi.

Both presented the paper on "Knowledge, Perceptions and Attitudes on Pap Smear Tests among Bruneians" during a session of the 9th Biennial International Nursing Conference at Universiti Brunei Darussalam yesterday.

"The knowledge on risk factors, prevention and symptoms of cervical cancer are poor and therefore intervention in increasing this knowledge need to be emphasised," said Muhamad Ibnu, adding that awareness regarding the importance of pap smear screening from both male and female population need further improvement.

They conducted a survey using a structured self-administered questionnaire to 368 people 165 of which are male respondents. All respondents were Bruneians or permanent residents between the age of 18 to 60 years old and mentally competent.

Mohammad Noh said that more than 10 per cent of the women respondents were not aware that pap smear screening is provided in the Sultanate. The figures of male respondents who were unaware were higher.

"People should be aware that pap smear screening does exist here and they should have adequate knowledge on the importance of the screening as cervical cancer is dangerous among women," Mohammad Noh said.

Both students agree that it is important to involve men in the effort to raise the awareness on pap smear tests as under family paternalistic culture, men play important roles on decision-making.

According to their survey, 20.8 per cent of women and 38.7 per cent of men are afraid of the screening test. Also, 49.5 per cent of women and 50 per cent of men said that unmarried women do not need to undergo a pap smear test. Meanwhile, 67.3 per cent women and 62.8 per cent men have the perception that pap smear tests can cause loss of virginity.

Both stressed on the need for married women to go for pap smear tests.

October 26, 2010 (Montreal, Quebec City) — Superman avoided kryptonite, one of the few things that could bring him down, but cardiac surgeons might one day willingly embrace another form ofKryptonite (Doctors Research Group, Southbury, CT), used to seal the sternums of patients who go under the surgeon's saw.

Kryptonite, a bone cement that bonds the breastbone after CABG surgery, improves patient quality-of-life scores and spirometry measurements, reduces pain and physical disability, and cuts the amount of pain medication needed after surgery, according to results of the Sternal Innovative Closure with Kryptonite (STICK) study.

Led by Dr Paul Fedak (University of Calgary, AB) and presented this week here at the Canadian Cardiovascular Congress 2010, the study showed that when used in addition to traditional wires that close the chest, the bone cement completely seals the breastbone within 24 hours and is designed to be integrated with native bone as the body heals.

"A lot of medicine is moving away from invasive surgeries to procedures that can be done less invasively," said Fedak. "But patients often stay away from invasive procedures to their own detriment. There is a lot that we can do with an open chest cavity. And with open-heart surgery, it's usually not the heart the patient worries about, but rather the long, painful recovery associated with cutting open the sternum."

Tested in 55 Stable CABG Patients

Bone cement commonly used by orthopedic surgeons is known as polymethylmethacrylate (PMMA), but it is a "toxic, synthetic product" that doesn't fully heal, explained Fedak. The Kryptonite bone cement, on the other hand, is a biocompatible polymer made of triglycerides, extracted from the castor bean plant, and calcium chloride. The product exists in a liquid form but changes within minutes to moldable putty, eventually hardening into a light, porous, and strong bonelike substance within 24 hours.

"What's also very cool about it is that it creates a regenerative scaffold where native bone cells can enter into it and lay down new bone," said Fedak. "It actually heals and allows for osteointegration. Those properties, and the fact that it bonds very specifically to bone and not other tissues, like the heart, lungs, or other soft tissues, make it ideal for cardiac surgery. Adhesives haven't been used like this before. People have used all kinds of things to promote healing of the chest bone, but nothing that will turn it into a rock-solid stable sternum within 24 hours."

Last year, Fedak and colleagues tested the strength of the cement in cadavers, showing that it was able to withstand increasing forces designed to simulate coughing and flexion of the torso. In this latest single-center study, the group tested the use of Kryptonite in 55 stable patients undergoing CABG surgery. As noted, patients reported less pain, required fewer narcotics for pain management, and had improved spirometry measurements. In addition, two functional scales--health-related quality of life and physical disability--were also improved in patients treated with the bone cement compared with controls.

"After surgery, traditionally, patients have some degree of physical disability and dysfunction, and then over weeks they recover and get back to normal. In cardiac surgery, that normally takes about six to eight weeks. With Kryptonite, we're seeing it around four or five weeks. Patients had less pain, better breathing, and recovery time reduced by a couple of weeks."

There were no reported side effects or adverse outcomes, but surgeons need to be mindful of the chest tubes running underneath the breastplate as the sternum is closed so that these do not adhere to the glue. Currently, the glue has been tested only in stable patients, and investigators are cautious about using the cement in unstable patients or those at high risk for postoperative bleeding, mainly because it requires more time to reenter the chest cavity if needed.

To heartwire , Fedak said that cardiac surgery has evolved significantly over the past 50 years, with much research into minimally invasive procedures, on- and off-pump CABG surgery, and even robotics. One thing that hasn't changed during this period of innovation, he said, is how surgeons close the sternum, noting that wire cerclage closure of sternotomy is used just as it was when the procedure was first performed.

"The wires themselves are not a problem," he said. "It's just that we're asking them to do too much over too long a period of time. With the cement, we're using the wires for what they're good at, which is bringing the edges of the bone together, but they're not good at keeping that bone together over a long period of time. Now, with the cement, we're only asking the wires to work for the first day."

UBD, BSP ink 5-year deal on occupational health course

Thursday, June 24, 2010 - A $750,000 agreement signed yesterday between Universiti Brunei Darussalam (UBD) and Brunei Shell Petroleum Company Sdn Bhd (BSP) will see the former stepping up its work in Occupational Health and Medicine, including new postgraduate courses on the subject and the establishment of a health centre for clinical research.

Through a Memorandum of Understanding (MoU) signed at the Pengiran Anak Puteri Rashidah Sa'adatul Bolkiah (PAPRSB) Institute of Health Sciences, the oil and gas company agreed to sponsor $150,000 annually for five years the UBD professorial chair in Occupational Health and Medicine.

"BSP's partnership with us marks a new collaborative dimension for UBD, where for the first time, a full professorial position has been undertaken by the corporate sector," said the institute's dean, who was a signatory to the MoU.

Dr Anita Binurul Zahrina POKLWDSS Hj Abd Aziz said that employment was no longer just about getting an income, but has "evolved" to consider much more than that, "adopting and transforming work to the needs and circumstances of man".

Now a worker's physical, mental and social well-being had to be considered as well as other factors at the workplace, where "a significant amount of our time awake" is spent.

In this regard, she noted that UBD recognised the importance of Occupational Health and Medicine, but acknowledged a lack of expertise in the field, citing constraints in human capability and available resources.

"Expertise in the field of Occupational Heal and Medicine in UBD is inadequate, and the appointment of experts in this field is clearly fundamental for the implementation of this important initiative," Dr Anita said.

"The role of an expert or professor in this field is crucial, not only for teaching, but also for conducting clinical research and development work.

The dean of the PAPRSB Institute of Health Sciences disclosed that the university will expand its curriculum to include Occupational Health and Medicine at the postgraduate level, apart from already offering diploma and undergraduate studies on the field.

"Indeed, the Master in Public Health Programme will be offered for the first time in August this year," she said. "Furthermore, we will also initiate more research programmes in Occupational Health and Medicine and in parallel, seek to cultivate young researchers through our Masters by research and PhD programmes."

"In addition, clinical work pertaining to Occupational Medicine will soon be set up with the establishment of our proposed University Health Centre, Insyallah," she added.

The MoU was signed yesterday by UBD Vice-Chancellor Dr Hj Zulkarnain Hj Hanafi and BSP Deputy Managing Director Hj Zainal Abidin Hj Mohd Ali.

On hand to witness the signing was Education Minister Pehin Orang Kaya Seri Kerna Dato Seri Setia Dr Hj Abu Bakar Hj Apong.

The deputy managing director highlighted that occupational health has always been an integral aspect of BSP's business.

"BSP, together with UBD, aims to promote and conduct research in Occupational Health and Medicine in UBD as well as in BSP. At the same time, (we aim) to provide a source of expert advice and community service for Occupational Health and Medicine for all agencies in Brunei," Hj Zainal Abidin said.

"Touching more on Occupational Health, this special branch of medicine will put greater importance of the health of our workers in their workplace. BSP sees the agreement as a good turning point that will pave the way forward," he added.
